Deciding on Dementia Housing Options

Figuring out the next housing option for your loved one with dementia and memory issues, can feel like an overwhelming task. It can be difficult to know if your loved one can live independently, need supportive living or a secure memory care environment.

This worksheet will allow you to get a sense of what the needs of your loved one is presently, and guide you towards better understanding what behaviours and signs to accomodate in the next housing option. This is not medical advise, and assessments from trusted health professionals should be used to determine where a person living with dementia and memory issues should live.

Once purchased, you will have access to the PDF and can download for printing. You can print single sided or double sided depending on your printer or at a local print shop at your cost. I recommend printing in colour but you can choose to print in greyscale!

If you do not have a printer or generally have difficulty with downloadable files – I recommend choosing a printed and mailed item to your country of choice. Product will be mailed to your billing address at standard letter delivery times.
