[VIRTUAL] Advance Care Planning and Green Sleeves

A Green Sleeve is your Medical Passport. It holds important legal forms and information about your health and well-being wishes and your goals of care.

In this workshop we will review why a green sleeve is important, explore what goes in it, and discuss how to use it effectively as an end of life/quality of life planning tool.

Your video and microphone will be turned off when you join, and you can decide whether to participate via video or just via audio.
