Deciding to age in place where you live takes planning. Knowing how best to support your health needs, your social needs and the service needs you will have is an important first step.
This worksheet will help you gather all the information you need (and your loved ones may need) to understand how you plan to age in place. On the first page, list your favorite housing options for when your health changes and home is no longer the appropriate and safe space. On the second page do a mini assessment of your home’s ability to keep you safe and independent. The third page is about your social connections, your activities you participate in and your social routine. The last page is your list of service providers you will call upon when their services are needed. Once purchased, you will have access to the PDF and can download for printing. You can print single sided or double sided depending on your printer or at a local print shop at your cost. I recommend printing in colour but you can choose to print in greyscale!
